  4. Pretty much 100% genetics. We are currently clueless at what most of our genes do, but if we could "read" our DNA we could probably predict when and how you will die, if it is a natural cause.

  6. Its because of the memories. Imagine if one day all your memories were wiped clean, and you were reset like a factory reset. At that point, death would make no difference because you have no identity or concept of self.
