  1. A fake surgeon has to worry about screwing up. A regular surgeon as long as he follows the 'standard of care' is pretty much immune to lawsuit and prosecution. So you don't have to actually worry about being good at all, just following standard procedure.

  2. They don't teach surgeons in medical school at all, it's all learned on the job anyway. Though obviously you need to know the human body as well and this guy also skpped doing a residency, which is where he would learn how to actually do surgery or at least the basics. Fun fact any doctor can just start operating on people, too. All they have to do is learn the actual operations they want to do, which just means they have to assist in the procedure at some point.
