  2. The university approving after is why they were banned. It was also bad because it wastes a bunch of people's time and is also illegal. They were lucky they did not get sued or even legal charges for the ethics violations.

  3. It seems pretty unlikely from a lone attacker. Maybe from a government but you can be sure there's backdoors in hardware already for them.
