  1. In this case it is the economy of scale. Population 2200 is going to be losing money because you would be lucky to see 200 customers a month and not enough go keep a full time pharmacy running. This is why major chain pharmacy isn't open there.

  2. It is not just US, rural town had its place back when everyone works agricultural jobs. Today we have automation in agriculture and most people don't work or live in rural town anymore. The decline in population means nothing would be sustainable, not just healthcare but school, fire department, roads, grocery stores, etc.
