I've watched a good deal of these movies at the lower end of the iceberg and here's my two cents: Even though frame-for-frame there is much more disturbing footage in the bottom films, it's often so nonstop and the plot is so nonsensical that you just become numb to it after awhile. On the other hand, more mainstream films like Oldboy and even the relatively tame The Wicker Man (1973) have a much higher potential to disturb because of how emotionally close the films are to the actual human psyche.
