Yeah I work for Blue Cross and the amount of claims I see deny for ridiculous reasons is frankly terrifying. They purposely turn their mailroom into a black hole that way they can deny claims by just saying they never got them and it's past the timely filing limit even when there's 8 calls documented by the doctor confirming and resending the claim. And the corporate structure of the companies are purposely so complex that nobody knows what they are doing which means it moves slower than government and there's no accountability for anything since every department just plays hot potato with an issue until someone finds a black hole to stuff it into. Insurance companies corporate structure is literally worse than crypto companies.

No, insurance companies can deny claims for whatever reason they want and a lot of times the system is purposely made broken so that claims deny for reasons that don't even make sense. And the ones that handle the appeal are the same ones that denied it in the first place so there's really no accountability.
