It's not just Cigna, this is definitely standard practice now. I work for a different of the giant healthcare corps and it's infuriating trying to even get obviously wrong claims correctly processed. You can only send someone's claim to be reprocessed so many dozens of times before you decide to put your head through the wall.

Most claims are not reviewed by a doctor or a nurse but actually the customer service rep is considered the "first line of denial" and they are paid minimum wage so the turnover is very high, most people only stay a few months. So if the system auto-denies a claim, customer service may deny it again as the 1st level appeal, then you are only left with the 2nd level appeal and hope to god that it actually gets reviewed correctly.

Yeah, doctors need billing teams that are funded like the mafia if they actually want to collect their payment. We really need doctors to start screaming louder about this, testifying to congress, going to the press, etc.
