  2. Many doctors/hospitals are now making you sign forms upfront that make you responsible for any denials. That's what it's come to. Doesn't matter if you have the best health insurance if they deny and you have to pay the full cost anyways. We are paying for the insurance just to turn around and pay for all our own care out of pocket anyways.
