I unfortunately work for a big health insurance company and I see patients claims denied all the time for either no reason or just obviously incorrect things. I've seen people have their radiation treatment denied when they are diagnosed with cancer. It's pure evil. And there is no recourse because the appeal just sends it through the same broken system to spit it back out denying for the same incorrect reason. And many of these systems are designed to deny as many claims as possible and justify it by saying "well they can appeal and get it corrected". But often the appeal process is very bureaucratic and most people don't bother (or don't have the time) to put hours of work into each claim that should literally be done for them as that's kinda the point of having health insurance to begin with. It's literally a full time job (probably even more) to continually call about insurance claims and try to understand the systems. And of course people call in angry or worse if they have a family member that's in a coma or dying and you can't discuss their info and they are having treatment denied because the insurance company wouldn't approve the prior authorization. And I don't blame the angry people at all, but I also got no power to help. I'm just paid crap wages to be a piƱata.
