They try to make the web as convoluted as possible so that not only are your claims denied and you have to pay the claims and the insurance company but if you want any claims to go through you have to spend 40 hours a week on hold trying to figure out your own medical billing in which all the codes and rules change every couple months. Not to mention that there are like 100k different medical insurance plans in the US. (Probably more actually)
