I cared for my late grandfather for years before his passing. He couldn't stand, but he could transfer, say to his bed to a wheelchair or his wheelchair to the toilet. I would grip his hand so he could pull himself up, then guide him to where he was going. If he would stumble or needed some extra oomph to get up, I'd often grip his hand tighter instinctively. He'd end up with a bruise that looks identical to what the president has. I'd suggest he either stumbled or he couldn't stand up and needed help up and someone who didn't know how to do it properly grabbed his hand too hard as plausible explanations.
