My late grandfather was quite frail in his last days. He couldn't stand, but could transfer, from a chair to his wheelchair for example. I'd grip his hand so he could pull himself up. If he stumbled getting up or needed some extra oomph to get up, I'd grip his hand firmer on instinct. That bruise on the back of Trump's hand looks identical to someone gripping his hand too tight trying to help him up. I'm suspecting he fell and needed help up (but we'd likely see other bruising that'd be more obvious), or he had trouble standing up from a seated position and needed a hand up.
