While I don't think it's actually syphilis, Donald Trump having wealth and private doctors to monitor such conditions well before the manifest symptoms, it would explain a lot about his mental deterioration. Most likely, it's bruising from a slip and fall or putting his hands out to stop himself falling. At his age, such bruising will show up as bright red.
