This was decades ago and a quack doctor that believed gender is entirely social advised having a gender reassignment surgery performed and raise the boy as a girl. When the child inevitably found out, tried to retransition back to being male, went into depression and committed suicide. His twin brother followed not long after. These surgeries are not and never have been routine among children. Gender affirming care in children are typically a name change and dressing as the gender they identify as. Sometimes puberty blockers are given followed by hormone therapy, but that's in puberty, so 12+. The only time "sexual mutilation" is happening is adulthood.
