To be fair to Trump, I doubt he remember exactly what he said about vaccines previously, so he doesn't realize he's contradicting himself, not to mention his fans will twist themselves into pretzels to make this sound like he's not contradicting himself at all. The only way I'd be in favor of a religious exemption for vaccinations is if the family in question is in some sequestered religious compound\community and does not interact with the rest of society. It's still a bad idea, but at least it wouldn't be affecting the rest of us.
