  1. When I took my police response and advanced driver course in about 1990 we were told to drive at 10% over and it was called 'Progressive Driving'. Never heard of the 3mph add on though. I would never have reported and driver unless they were 10mph over but more often than not it would be just a warning. Making it is ridiculously tight will only take a driver's attention away from the road to the speedo and that can be dangerous.
