Go to a public hospital in Shinjuku and it's full of elderly people, although that's rather a business area. So imagine anywhere else with more of them and less healthcare facilities. Even independent physicians who work up to their 80s give up when they can't afford to upgrade due to new policies. Old folks are unlikely to move already, but that must be another major concern for them. For the working class, being homeless isn't an objective. I assume they hope to find actual job opportunities in the big cities. If I have the money to buy a residence in a rural city when I retire, I'll totally do that.
