  4. First off it was an ELECTIVE SURGERY. Second this guy probably has 40 or 50 people on his personal staff. So even if he couldn't the Deputy Secretary couldn't have notified The White House? Additionally we can also assume that the Chairman of The Joint Chiefs didn't know either.
