  1. It's not. She's supposed to be referred to as the Duchess of Sussex, but the media hasn't gotten that through its collective thick head. And considering that the media still refers to Catherine by her maiden name after eight years of marriage and three children, I'm not holding my breath that it'll be getting Meghan's title correct anytime soon.

  4.  @nydabeats  Right now, neither since, being titled, she doesn't use a surname. If, however, she and Harry decided to chuck it all and become private citizens, they'd have the surname of Mountbatten-Windsor.
