Transsexuality exists of course since thousands of years, since it is something you are simply born with.
For the most time there was simply nothing you could do about it.
A big problem of it is by the way not only the biological aspect but especially the cultural/social one, so the "gender", which is everything that goes beyond the biological sex, beyond reproduction, for example that men are worthless tools and can be wasted in wars or dangerous/lethal work. That's for example a social role, a gender role. Blue being manly and pink being girly, trousers for men and dresses for women, that men should drink a lot of alcohol and women better not, because not ladylike, that women should be emotional/caring, while men should shut up and eat it - and so on, all that got nothing to do with the biological, but the social gender, which of course differed in time and place of human history, though most of the time men were seen as expendable, well, poor men (so the majority).
When it comes to historical figures, we in the end know too little. Most of what we know is second source at best, what is often enough a lot of bullshit, just as it's often enough already bullshit when you read something in media about a person - and that without thousands of years added to it.

Uneducated, because they got no clue what they are talking about, what would be fine.
But also dumb, because they still talk about it and that without caring to educated themselves .
That's of course nothing special regarding this topic, dumb people are like that in general.
Biggest problem: that level of stupid even makes up the idea that they aren't dumb, no, in contrary, that they are very smart and know things just like that - or because someone told them on the internet, mainly other dumb people.
It's like a stupid gangbang, they come together, fuck each other and multiply their stupidity with it.
It's sad, but worse, also dangerous, because this is easily abused by evil people who love to manipulate the stupid - for example like that, by distracting them from the real problems and their own crimes with making them hate minorities over nothing.
And they don't get - again because the dumb and stupid part. You can't even explain it to them, because they will simply deny to get smarter, since in their delusion, they ARE already the smart guys, even though they ruin their own and everyone lives by falling for the evil guys - and that since thousands of years, that's also nothing new.
Without the stupid, even extremes like the Nazis would not be possible. You don't have enough psychopathic evil people for that, you need the stupid cannonfodder.
Oh, and don't get me wrong: you got stupid on the side of the minorities, too, who are also exploited the same way.
Those people the mentioned stupid quote? Same thing. There are some evil peopel who fake it on purpose, but those who follow it and run around spreading bullshit that only hurts their own positions, they are also dumb, because they also don't get it and don't want to.

This doesn't mean, that someone who is not happy with biological sex or social gender (that's not the same thing by the way), must be transsexual. Especially the gender role of men is so devastating, that it killed millions of men in history in many ways, billions when you add the fucked up idea of war to it, which consumes males lives like tinder (not the app) thrown into a vulcan. So it's no surprise, even though it is part of the gender role of men to just eat it and stfu for men, that there are many boys and men unhappy about it or getting depressive. Same the other way around of course, even though the female gender role is less brutal and was much more liberated in the last decades.
But being unhappy about your fucked up gender role doesn't make you trans of course.
There aer also homosexuals who - also pressured by hate against homosexuality - may think if the like men and that this is unnatural, that they should be women, than everything would be fine or if a lesbian, than they should be man, so it would be heterosexual to love women.
This if of course wrong and will not solve their problem, probably make it worse, because now they kind of made themselves transsexual by changing their body on top of their other problems.
And being transsexual is obviously nothign anyone wants. It clearly ruins your live, when you whole body is wrong, finding love being close to impossible, because you can't find the right partner, because YOU are the wrong partner, same for children, where it is much harder for transsexuals to be the father or mother their soul wants to be, but their body can't deliver.
In the end it's like any other problem you are born with.
Would you freak out about someone born without legs, that using the possibilities to give them legs is wrong?
I mean guess what: if you be like that, ALL medical care is wrong. It's unnatural. Because nature wants you to die when you get ill or hurt, that's how evolution works, killing you off.
And guess what: it's obvious that neither homosexuality nor transsexuality got anything to do with evolution because if it would, it already wouldn't exist anymore.

@Пинагод I mean, 0,00001% or so of peopel in history were emperors. And somethign around <0,1% are transsexual (this isn't about the exact number).
While both can come together, it's of course just unlikely, though clearly not impossible.
The bigger question would be if you would find anything about it in the few sources we got about that time.
While an Emperor might be able to live more freely than others, it's still unlikely that he would tell others "I'm actually a women" - especially in these times.
They also wouldn't have believed it. We talk about times, were humans believed in a a lot of bullshit. So they might have linked it to the gods, but then for example someone like Zeus would have taken a woman, change her into a man to fuck with the Romans for his amusement. (yes, they got such stories that gods do that).
