Although you are correct that liquid hydrogen is a nightmare to handle and contain, there are no "Apollo fires and explosions" dues to an escape of either liquid oxygen or liquid hydrogen. The Apollo 13 abort was caused by exposed fan wires shorting during the 'cryo-stir' and the teflon insulation catching fire in the pure oxygen environment. This fire rapidly heated and increased the pressure of oxygen inside the tank causing the no. 2 oxygen tank to explode. Challenger was destroyed due to a failed O-ring seal on the right hand SRB which had become brittle in the very low temperatures on the launch pad.
