  1. The definition of 'failure' is objective and subjective, in the end. Some cracks are 'indications' to be tracked, others COULD be failure. However, unless you have an evaluation by independent technical experts with owners' cooperation, you can't cry 'failure' of an entire structure on mere quantity. A Quality evaluation is needed. Worked in Quality Assurance in nuclear projects, and it's not a simple process in a lot of cases.
