  6. Not necessarily. Health and Safety will often come to an "agreement" with a company if the breach is not thought to have been deliberate. The aim is to improve safety rather than punish.

  7.  @KingoftheJuice18  I am trying to point out why they did not think to send anyone to look at the engines and why, without the benefit of hindsight, their decision is understandable. Firstly, their initial indications (and previous experience) suggested that the RHD engine was at fault. Their initial actions appeared to work, "confirming" their conclusions. Their working practices at the time led to them having a high workload. It is somewhat simplistic to say "They should have done...." because hindsight has given us an insight they did not have. Of course if they had sent someone to look at the engines, in this case, the accident would have been avoided. The important lesson to learn though is why they did not think to do it.

  8. ​ @KingoftheJuice18  My understanding is irrelevant, what matters is theirs. The video clearly explains the factors that led to the decision to ignore the vibration sensors in favour of other inputs. As pointed out elsewhere, a classic example of human confirmation bias.

  18.  @SJF15  Ffs. The point of this thread is that the pilots THOUGHT they had done everything right. They believed this because they had succumbed to "confirmation bias" whereby the responses from the aeroplane to their actions appeared to confirm their diagnosis of the problem. This accident is a good example of how a natural reaction to an emergency can cause the unwary to make incorrect decisions. Why do commenters find this so hard to understand?

  20. As a non pilot with only an interest in aviation, I recognise that a combination of rising angle of attack and falling airspeed is a sure sign of an impending stall. Why do you think it is that we see so many examples of trained pilots not recognising that their aircraft is approaching/in a stall, even when the stick shaker operates?

  23. So that was the cause. Excellent video. I met a few people who were on this flight and from what the told me, I would have likely suffered a brown trouser incident!

  24. Thanks for the explanation. I had always assumed that the crash was down to a change in the cg as the load moved, just goes to show why speculation is a dangerous thing.

  25.  @peterwone7001  I am not so sure. Riders and drivers react well to collision avoidance it's true, but that's something we do as a matter of everyday life. We don't bump into each other when walking as a rule. I was talking about the split second diagnosis of unexpected technical issues. "What has happened, how will it affect the aircraft, what do I need to do immediately to get to a safe state?" How many flights have ended safely because a pilot, while processing all of this information, has also flicked a switch or changed a control setting that has proved crucial?

  29. Excellent as always. Have you ever looked at/reacted to the videos distributed by Approach and Departure Videos? I have no experience of aircraft piloting, but as an observer the captain appears to offer an excellent environment for a first officer (or even a flight simmer) to gain experience. I would be interested in your thoughts.

  32. Good to see you back. I love your approach to these incident videos, safety as it should be. Regardless of what is said, a lot of safety investigations are still focussed on appointing blame rather than understanding the why.

  34. Why is oxygen still available on aircraft? I can see a possible reason for a supply for emergency medical reasons, but for pilots and crew would an air supply not be preferrable? Sure, there might be weight considerations (although modern COPV's are much lighter than steel) but...........

  36. Excellent video and excellent performance by the crew. For a spell back in '91/92 I used to check test filters from the fuel bowsers on RAF airfields to monitor for potential fuel contamination. This incident certainly puts that job into perspective.

  51.  @MentourPilot  Thanks for replying. I guess that raises two further questions. Why do pilots fail to recognise they are in a stall and pull back on controls when the aircraft is telling them they are already pitched up? Would it not be really helpful to pilots to have a clear representation of their angle of attack, perhaps displayed as a visual of the aircraft relative to a horizontal line? I'm sure it's not as simple as that though. If you have not already, could you perhaps produce a video explaining why pilots fail to recognise that they are in a stall?

  59. A mini one. They compare cabin sizes with a King air 350, but even using a Falcon 50 as an example its wingspan is approx 19m, compared to the 55m wingspan of Thunderbird 2. I know, I'm nerdy :D
