You can survive IF (note the IF) you are under a few feet of earth like a bunker, for 48 hours +/- (depending on distance from the blast). Also true that you might survive if you are over a mile away from the blast zone, AND IF you are upwind of the fallout with “survivable” radiation ☢️ sickness. However as motor cities and government centers would be hit, and HALF of the population lives near (90 mile radius) or in major cities, you would potentially be talking about 100 million to 150 million lost in the USA. This does not include the casualties from those in the path of the fallout from the blasts.
