Mh I think also the ATC controller made a hard mistake here. He let the KLM captain not reading back his "standby for takeoff". I think he definitely should have made sure the KLM received this, more so when he should have noticed his impatient mood. An error not as severe as the KLM, but ... not much better. Wonder why this wasn't mentioned in the final report?
