Once again in the USA, "Why do we need Health & Safety regulations & enforcement?". The inevitable disaster then the response is get a lawyer & blame someone else.

Rather like the DC-9 or was it 10? This was a major design flaw with an outward opening door. You was a fail-safe design which is what an inward opening door provides i.e. if a latch fails then the door remains shut whereas the outward design fails with the door open & explosive decompression. It is a mystery how these designs passed an airworthy safety test

F16 pilot at fault! He collided with the tail of the C130 & ejected of the airfield without any thought where the aircraft would crash therefore both negligent & selfish.

No charts, no compass, no visibility & the captain taking a break at the wrong time. Accident waiting to happen. Typical USA where safety is last if it effects the bottom line.

@gnarthdarkanen7464 That was some reply. Actually it was the switch to Flint river water & lack of proper treatment the caused the problem not lake water.

So many small failures led to this tragedy: Captain did not understand the watchman rule & no one checked that he was implementing it correctly; the safety briefing not being correctly performed due to the "interruption"; obviously the smoke alarms didn't function as not 1 passenger woke in time to get out; very bad practise to leave any devices charging overnight especially in a confined space like a boat.

There is white & blue asbestos. They are all silica minerals which cause the damage. I lived in a flat with an old giant storage radiator to provide poor warm air heating. When they installed gas central heating it was left in place due to asbestos & the huge expense of removing it/them across the whole estate.

This was a cover up to hide the negligence of the water company. That something as a relief driver caused this problem shows that proper safeguards were not in place. That tanks didn't have clear signs on them is 100% negligence. The £10,000 was a joke given the multiple failures.

Fines in the USA for safety violations are small & the company usually fights them to get them reduced. Profit is more important than workers lives in most cases. The Bacardi case they only got a fine if $192K for 12 violations

It was the Oil Crisis of 1972 that made it uneconomic plus the USA banning it from flying across the US to LA, the other countries followed suit thus the routes were confined to over sea to coastal cities.

There was another of a woman climbing over the safety barrier that had warning signs on cliffs near Sydney, Australia. I saw another video that said there is about 4 deaths a year of people falling into the Grand Canyon. You can't save stupid. It wasn't a selfie but a woman tried to save her dog that ran off & fell into a thermal pool in Yellowstone. That should be the topic for another video: people killing themself trying to rescue pets.
