Good video, good analysis. This was no accident, this was months in the planning but the motive will very likely remain a mystery.

Interesting and more detail than another video I had watched about this accident. To me it was bad training: captain did issue clear instructions before going for his rest, the first officer was freaked out by St, Elmo's fire & the relief pilot never took over soon enough and was unaware that the panicking FO was still trying to fly the plane instead of releasing control to the relief pilot. That they didn't follow procedure when the instruments stopped working meant that rather than be calm and assess the situation lead to disaster.

I am old enough to remember this. This video is by far the best explanation of what happen. KLM Captain in far too much of a hurry. At the time it was reported that Spanish ATC were not well trained & their English skills were average at best. A bad decision to refuel the KLM as that had a bearing on the fireball that engulf the 747 and shows again that the KLM captain was too focused on the return to Amsterdam and not thinking about the current situation.

I may have missed it here but another video on this accident said there was nothing wrong with the nose wheel, the bulb had failed in the indicator. So rather in the same vein as the saying "for want of a nail the ship was lost", here a faulty bulb on a relatively new aircraft caused the crash.

Cascade effect. 1 problem causes another. Airbus & maintenance not fixing the problem quickly. FO not flying the plane but concerned about the captain & not asking for help from the cabin crew. Inadequate conversion training as stress means reverting to old behaviours

This check flight shouldn't have been done in these weather conditions plus the instructor did a very poor job of being the FO. It was as if he was more concerned with the exam than making sure the flight was conducted correctly & safely.

Old enough to remember this! Poor design, inadequate fix & MD being too cosy with the FAA who should have issued the AD. Surely then the problem was resolved? What's that Boeing, FAA & 737Max. This accident shows that fines don't work because airlines just get the money back from tickets/cargo fees. The people who signed off that the 3 fixes had been done should have gone to jail. For years after there were many who were reluctant to fly on the DC 10.

US English can cause problems due its lack of precision. United should have accepted the late change without the clarity that was missing: ask a question & expect an answer, if you don't get it ask again. I like the phrase "Skill fade" or as I would say you get "rusty" not using a skill regularly.

A ridiculous nonchalant approach to a serious issue. Formaldehyde can do serious acute damage to the respiratory system. That a software update & a change to switch off the cockpit A/C from the left engine at take-off seems to be ignoring a potentially catastrophic problem that could result in the loss of the aircraft & all on board.

Boeing management tried by the Keystone Cops. You would think after the MCAS debacle things would change but here we are again with important info withheld from operators & pilots.

Poor management & maintenance, poor training & once again a Captain over-confident and complacent. Flight crew should not have accepted the change of runway. No reason why this was the 3rd flight where the captain was PIC and no doubt his inability to allow the FO to fly a leg contributed to him being overworked. The fact that he was diabetic and not being properly monitored by a company doctor is also negligent.

Enjoying these in-depth analyses. In safety critical situations discretion can be dangerous. The Captain taking over control didn't help neither did his military background ( less risk adverse & maybe concerned what the 3rd pilot would say to a go around)
