My grand-uncle developed it quite late in life, at some points he even started blaming his own children and sisters of spying on him, even if they were all the way in Latin America and he was in Westen Europe. He also believed that his neighbours and later caretakers were against him. He is slightly better now, he is in an insane asylum and sometimes he seems more normal (but never "the old him") and sometimes he seems worse off.
