  1. It is a shame that these things keep happening, and we don't learn from them. People have mentioned the space shuttle incident. Another almost identical incident to Titan but with airplanes was Alaska Airlines flight 261. Not much learned from that incident either. (I'm not using the word accident since just as with Titan it wasn't an accident, it was a predictable outcome). Which is why it keeps happening. When those responsible are never held to account, there is no incentive for them to do the right thing.
