The US already has an issue with hospitals shutting down in rural areas, and most of the latest shutdowns occurred in states that didn't accept federal Medicare assistance. You're also talking nonsense about positive rights. If you have a right to a speedy trial, who's going to judge that trial? What about the right to an attorney?
