  1. David, stop with the extreme 5 billion people per $100b nonsense. Revenue generated would be per capita. If you have a developed country of 5 billion people, you could generate more revenue than $100b. If you have a developed country of 5 million people, you don't need to generate $100b in revenue. All other developed countries have both lower per capita costs and better outcomes.

  2. Also, how is saving 68000 lives not an increase in quality, as well as access? If you're getting zero access, then you're getting zero quality, until you need emergency care. You're just leaving millions of people with no, or limited, access out of the initial quality metric. So, what metric are you using for quality, if not overall life expectancy, infant mortality, maternal mortality, etc.? Shouldn't the quality of healthcare measured in its capacity to keep people alive, first and foremost?
