I personally take a "should" as a "should if safe to" So in the scenario of coming off an island, if I see pedestrians, I will check behind me to ensure I feel confident I can stop without someone hitting my back-side. If I think someone is following too close, I'll actively position to signal to the pedestrian that I'm not going to obey the highway code "should" rule. So far, I'm yet to see that fail.
