Not everyone is constricted to observe Ramadan, such as if they are ill or really don't want to.  Yes, however, during sun they should be given pure water to rinse their mouths with and spit out neatly.  Beyond that, I suppose they must be force fed when there is no sun?  They probably shouldn't be in there long, however.
