They measured the frequency of violent deaths in primitive societies. As it turns out the average is equal to the worst case during WWII. About 20%. What you mean I guess, is that in recent years killing civilians became a bit more common. That could be true, but I doubt it. Sacking cities was the norm, ethnic cleansing was too. On top of that soldiers used to die much less back then, so I'd expect it all evens itself out.

So it's supposed to be complicated, but I don't see how it is. The simple model still works. If climate change caused droughts and famine because people were not able to grow crops, everything else would follow from this prime cause. Everything else apart from earthquakes. It those happened to a large enough scale, then it complicates the simplest possible model to a degree. But it does not mean that civilizations wouldn't collapse without earthquakes. They could go down anyway, or earthquakes could happen and have negligible impact.
