In the Maine's case though; it was in part due to having to pack in a lot of stuff on a relatively small hull, shile still wanting fore & aft firing arcs. The munitions stowage though... yeah~ that was definitely a costly mistake.

The root cause of all these issues?... yes it's our age old friend (sarcasm), The Politician . Or in the US's case specifically: Congress , Noun , the opposite of Progress.

@simonwaldock9689 Yup. Coal dust in a compartment - though the proportions is a factor - is one of the oldest examples of an accidental fuel air bomb. Tis one of the various reasons why Railway & other types of coaling stations tended to damp the coal with water, so as to reduce the dust as much as possible. (besides the fact that workers dying in [as then untreatable] agony from injesting coal dust makes for terrible PR)

A couple of german (WWI era) ships would also like a word XD . That said: tis relatively rare though.

@kpdubbs7117 Ark Royal (WWII) also had several german claims she'd been sunk by the time the axis actually pulled it off, though she also went slowly. (though unlike Yorktown she might have surrvived yet again, without the mix of human error & bad luck that meant she eventually went down)

Eeh. The Coles Turret is generally regarded as the 1st use of rotating gun turrets on a warship, even if the type of turrets most think of today were derived from the Armoured Barbette.

Eeh. Dictators or absolute Monarchs often need a reason too, as if the populace or the ruler's keys to power don't support the war, they risk either being deposed, or an insurrection.
