There is a channel called Idaho Bariatric and Metabolic institute and one of the doctors there say that it is often the opposite, just like those with type 2 diabetes often have high levels of insulin. And this make sense if the natural release is short lived but a person that have "blunted" their sense of satiation and eat often (and chose food that tend to make you eat more by relaxing the stomachpoach, like sweets). If they are "grazers" that munch on snacks continuously trough the day it would be a continuous release? Now, I believe they do bariatric surgeries so there might be some bias but he said he is not keen on his patients having these medications to help lose weight to get to surgery, basically for the loss of lean mass. Especially people who are a bit older, or women in menopause. If they are morbidly obese and lose 100-150 lb and 20-30% of that is loss of lean mass (muscle and bones) they will not be able to replace that lean mass. And they be much worse off if they gain back the weight then because that will give a much higher fat percentage compared to before the treatment even if the weight land on the same level.

I think that the psychological factors are often "missed"when people have very difficult to adjust to a healthier diet. Be it a real eating disorder or a tendency to make poor food choices when stressed in some way. High cortisol can have people going for very palatable food, idk if it is a function of survival instincts that may be a false alarm in a fight or flight mode in a way that anxiety can present itself. Mental problems have gone in the similar pace as the increase in obesity rates and the consequence of various chronic illness of said obesity (and likely adding to the stress people may already have, not just worry about poor health but also the negative economic outcome of being unwell).
