Because institutions aren't an omnipotent being, but a group of people. Many are naive, others committed lesser sins that do not amount to fraud, others are more concerned with politics than results (most major fraudsters were DEI hires), and many are also fraudsters and don't report crimes except when they have a personal vendetta.
