@Anarchistarchitect ahem, casualties per million miles traveled places airplanes (consults google) 62 times safer. The reason is first airplanes go faster but more importantly the discrepancy is almost totally due to drunks and road rage and that is something everyday people can understand, anticipate, and control but technical breakdowns simply kill people off without apparent cause so airplanes are SCARIER, not lethal. As the blacksmiths say, no one burns themselves on red hot metal, they burn themselves on metal that looks safe.

@j.edward4379 Lose the hyperbole. There are perverse incentives but the profit margins for cost+ is a fraction that of fixed cost. I.E. Building anticipated to cost 10 million to build, so cost+ contractor asks for 11 million, while fixed cost contractor asks for as much as 20 million because he must make up for losses due to canceled programs. The only way to fix this is "verticle integration" which is corporate jargon for cutting out the middle man.
