For the mystery third pocket. It may be that combat ready soldiers were so thinly dispersed and so disorganized that neither the Germans nor the Russians could tell where the German lines were. This isn't uncommon in these battles since throughout the war there were all kinds of unorganized pockets as reality never follows clean lines on a map. The 28th Jan offensive may have put a Russian unit between German units, but the German command didn't even notice.

It's more likely Schmidt and Paulus were both working as independent commanders using whatever forces they could get in contact with. As you said, the unit markers simply show where the bulk of the men generally were, not any organized units or lines.

There was a revolt in Chechyna in 1942, but the skirmishes in the far east were continuations of the Russian Civil War and those wrapped up just before Barbarrossa.
