You should start a preventative health care clinic for spinal injury. Something like care you could offer to avoid a surgery. It would likely pay much less but you could use your education and desire to lessen suffering to help people which clearly is something you aspire to do. You could still offer people what you intended to offer in the first place with a perfectly clear conscience. You could also start a general practice coop type organization. There are more and more of those popping up all over the place. You wouldn't be reliant on insurance because your patients would all pay a monthly fee and you would care for them as needed. You still have a lot to offer people and could use your experience to do that very well with focused attention.
