If someone were facing a 15% capital gains tax on his unrealized bitcoin gains, he should just go ahead, sell the bitcoin, and pay the 15% tax. But if someone really were facing a 40% tax on his unrealized bitcoin gains, what might work... at least in principle... would be for him to get together with someone from a country that takes a "hands off" approach to cryptocurrencies, and to then sell the bitcoin to him (or her) for 90 cents in the dollar (or some other amount that is agreed to). That other person could then convert the bitcoin to fiat (and realize an instant 10% gain). Yes, there's a CHANCE that federal agents would track down the blockchain records (and the various bank records) and figure out what happened; but they probably wouldn't bother unless they had motivation to target a specific person.
