  1. There is also the Therac-25, although the bad software of that machine was allowed to act very poorly due to the absence of hardware interlocks. On previous machines, the faults were hidden by the presence of interlocks, that didn't allow any deadly condition to occur (say, the lack of a target to convert electrons to X-rays, when in high energy electron beam mode).
