  1. Obesity is well known, other factors you mention might make a small difference but getting to a reasonable body weight is the number one thing.

  2. Small pox, Hepatitis A and B, HPV (most common variants) , the plague, measles , polio. Actually quite a lot of them. You can say they aren't completely gone which is correct but vaccination or in the case of the plague detection, the risk of these is low even in low income countries and extremely low in high income countries.

  3. Actually there is a study on that and technically it would appear to be correct. Riots so far have achieved the best lockdown for the masses. When you see rioters on the streets burning down buildings and being violent to passers by in some cases there is a good reason to shelter in place. So it spreads through the protest mobs but reduces infections in the general public.
