I know nothing about insulin pens but 25 times (five packs of five) 3ml pens containing 100 units per ml costs $183 AUD in Australia free market price including postage, about $140 USD. Product is 70/30 mix by a company called Novo Nordisk, brand names for possibly the same products seems to vary by country. Tried searching for an open market price in the US but they just have phone numbers to call and ask.

For generic drugs you are probably right. Public run hospitals are almost universally cheaper then privately run hospitals and I think the same would likely apply but due to scale would need to be multinational agreements, it's not viable to produce a whole manufacturing process for a drug that is only £1 million in consumption per year but if say Australia produced that for all the countries in the agreement and UK another few drugs etc. it could work. Rock solid agreements with original manufacturer would also be ok but need more than one manufacturing plant in different locations due to things like weather and supply chain events.
