I think people are missing the main point and that is that doctors and nurses are under a lot of mental strain, as he said normally he would take it in his stride but on this occasion he is basically admitting he had thoughts of I don't want to treat this patient. He didn't act on the thoughts but having the thoughts shook him up a bit which is why he basically told others.
