Medicare has the exact same rules, where do you think insurance companies are allowed to do it, Because it was actually started as a Medicare regulation to save Medicare money Every morning on rounds the Medicare rep would leave a sticky note that by CMS guidelines I had to discharge the patient or contact the Medicare rep to justify why I felt the Medicare patient was not ready to go home Please don’t comment on topics on which you have no knowledge

As a recently retired physician, a 23 hour observation stay is very different than a 24 hour admission The cost that the surgeon and hospital is allowed to charge is significantly different We did 23 hour short stays all the time, sometimes we might observe them for 6 , 8 , 10;hours and release them. But you get the same fee. If the patient was initially a23 hour short stay but wasn’t ready to go, we had to change it to 24 hour stay and justify why because it’s a big charge increase. Think about it, insurance companies want as many 23 hour observation stays The Hospital wants as many 24 hour + stays. Both parties have financial incentives

@ because the commentator was erroneously promoting that Medicare “ for all “ is the solution. 30 years in practice, as much as I hated dealing with insurance companies, Medicare is worse How much does Medicare pay if you fall and an ambulance is called to take you to the ER and you have injuries but are able to go home. Answer. Nothing. The patient is responsible for the entire charge
