  1. It's more subtle. The doctor says what the patient needs, but because we have to pay so much for it, the insurance company replies with how much they will cover. Mind you, you will have to pay hundreds per month for this coverage AND you will always have to pay at least $20-30/visit. Then when the doctor says your doctor's opinion is not covered, you can come up with thousands to buy the treatment or go with out. So many surgeries and tests are denied as "optional" or "excessive" and ppl die bc they cannot afford to pay thousands.
