  1. It appears that the helicopter directly struck the 1,200 ft radio tower below the red warning beacon. The helicopter pilot was probably flying VFR on the clear night. The helicopter was flying too low below the known radio tower marked on navigation charts and using landmarks to fly.

  2. It appears that the tower lights on the structure were not working, but in the video, you can see the red beacon light going on/off. The helicopter pilot would have been warned in the Notice To Airman (NOTAM) that the obstruction lights wasn’t working prior to his flight if he check the NOTAMs.

  3.  @JasonHrncir  The reporter was right, there was a NOTAM on the non-working obstruction lights, the top red beacon light was working. A radio tower has other lights on the structure to outline the shape of the structure at night for pilots instead of just a single blinking red beacon.
