On a ship there is a standing watch on the ship’s bridge 24/7. Even while at dock, the bridge is the center of control and communication of everything going on within the ship and around the ship. The standing watch at the docked ship should seen the impending danger and have blown its horn to the danger. Both officers standing watch of the dock ship and the reversing ship will have a black mark on their records

@-DM The standing watch on the dock ship noticing an imminent collision is about to occur would have hit the ship's horn warning the other ship and activated the ship collision alarm to warn the ship's crew below deck to brace for collision.

@djrampantuk If this was a mechanical failure the reversing ship would have blown his horn warning other ship he had no control of the ship and prepare for a collision. It was negligence on steering the ship.
