Trump was talking to Ivanka who was behind Melania, and Melania initially thought Trump was talking to her, but then realised he wasn't and then did a sad face. This is what happened, as i saw an enlarged image of this. It's not really a big deal though it does show that Trump it does echo the widely held belief that Trump cares more for his eldest daughter than he does for his wife.
